Subscription Services

Your membership in a GEVITIX LAB subscription program ("Subscription Services"), including your ability to access the services, begins when you complete the intake form, at which time you will be charged for the first month (or first plan period) of the Subscription Services.

Subscription Services include access to independent licensed providers, medication where clinically appropriate, baseline labs where included in your plan, and 24/7 online support.

If you are disqualified by a medical provider for any medical reason, you will receive a full refund.

There is no guarantee of results or effectiveness from participation in the Subscription Services. Individual results vary.

Cancellation Policy

You may cancel your Subscription Services at any time, for any reason. To cancel, contact customer service at fc@gevitixlab.com or use the chat feature in your patient portal.

To ensure a smooth process, your cancellation request must be received at least 72 hours before your billing date. If that notice window is not met, your subscription will be charged on the next billing date and the cancellation will take effect on the subsequent billing date. After cancellation, you will continue to receive the Subscription Services through the end of your current billing cycle.

Refund Policy

Refunds are available for any reason, except for any month in which medication has already been ordered on your behalf. Once a medication order has been placed with the pharmacy for a given billing cycle, that cycle's charge is non-refundable.

If your Subscription Services are cancelled due to disqualification by a medical provider, you will be refunded the remainder of your Subscription Services charges following the disqualification.

Federal law generally prohibits the return of prescription medications to pharmacies for refund or reuse once they have been dispensed to a patient, in order to protect public health and safety. This ensures that all medications are handled and dispensed with care and in compliance with applicable law. Approved refunds are issued for your most recent billing cycle only and will not be issued for past billing cycles. Consultation fees are not refundable once a consultation has occurred.

Damaged or incorrect items. Inspect your medication immediately upon receipt. If it appears damaged or incorrect, contact customer service immediately at fc@gevitixlab.com. Damaged items may be replaced by the pharmacy upon receipt of evidence of the damage. Incorrect medications will be replaced by the pharmacy.

What is included in my Subscription Services price?
Your Subscription Services price is a single price for the plan purchased. Depending on your plan, it may include consultations (live and via electronic form review) with an independent provider, medications fulfilled through a partner pharmacy, lab services, care support, a 24/7 patient support line, and other services supporting your treatment program. The specific services, labs and medications covered by the single price may vary by plan.

What if my provider determines I am no longer eligible?
If your provider determines you are no longer eligible for the Subscription Services, your subscription will be cancelled and a refund issued for the remaining charges following the determination. For questions about an eligibility-initiated cancellation, contact fc@gevitixlab.com.

Unless otherwise stated here, the GEVITIX LAB Terms & Conditions govern the Subscription Services and this Cancellation & Refund Policy. In all events you remain responsible for all fees for services and Healthcare Services rendered; this policy governs Subscription Services fees only.
